REPLACEMENT OF LOST/MUTILATED/DAMAGED PASSPORT

BASIC REQUIREMENTS FOR REPLACEMENT OF LOST/MUTILATED/DAMAGED PASSPORT
 
HOW TO APPLY
  1. Schedule an Appointment: Log in to https://passport.gov.ph/
    • You will receive your Application Form with the confirmed appointment.
  2. Prepare your documents. Complete your documents before your appointment. Complete your required documents  before your appointment (i.e. Printed copy of application form, et al).
  3. Proceed to the Embassy. On the date and time of your appointment, proceed to the embassy’s consular section and bring your documents for submission, further evaluation and payment of consular fees.
  4. Biometric Data Collection. Your photo and other biometric data (ie. fingerprints) will be taken digitally.
  5. Passport Release/Collection. Your passport can be collected in person or can be mailed to you for an additional fee.

WHAT TO BRING

  1. Passport Application Form
    • Printed Passport Application Form  - You will receive in your email the computer generated Application form with your appointment confirmation
    • Book your appointment here.
  2. Latest Philippine Passport
    • (IF LOST) One (1) photocopy of the Philippine passport data page, if available
    • (IF MUTILATED) Original and one (1) photocopy of the Philippine passport data page
  3. Birth Certificate (IF LOST PASSPORT IS NOT AN E-PASSPORT)
    • One (1) original and one (1) photocopy of the birth certificate issued by the Philippine Statistics Authority (PSA) on security paper.
    • If you do not have your document issued by the PSA on security paper, please order online through psahelpline.ph or psaserbilis.com.ph or if you prefer an e-apostille copy, please order through www.apostille.gov.ph
  4. Proof of Philippine Citizenship
    • One (1) original and one (1) photocopy of proof of current Philippine citizenship, such as resident ID card or dual citizenship identification certificate
  5. Notarized Affidavit
    • (IF LOST) One (1) original notarized Affidavit of Loss
    • (IF MUTILATED) One (1) original notarized Affidavit Destruction/Mutilation
    • The Affidavit will be executed at the Embassy.
  6. Police Report
    • (IF LOST) One (1) original police report
  7. For Foreign Passport Holders
    • Original and one (1) photocopy of foreign passport data page (if available)
  8. Payment
    • Euro 150.00 processing fee
    • Accepted payment methods include cash (exact payment) or  credit/debit card 
Note: For applicants who have copies of their lost ePassport (with chip logo), the PSA-issued Certificate of Live Birth/Report of Birth and valid ID will not be required. However, additional supporting documents may be required, as necessary to ascertain citizenship, identity, change of name for married women etc.
