REPORT OF MARRIAGE

Strictly By Appointment Only.
 
Requirements:
  1. Accomplished Report of Marriage Application Form.
  2. Uittreksel uit de huwelijksakte / International version (4 languages) of Marriage Certificate from the Gemeente. The Marriage Certificate must have an “Apostille” from the appropriate Dutch District Court or “Rechtbank”.
  3. Valid passports of husband and wife, and copies of the passports’ data pages.
  4. Residence permit of the Filipino national.
  5. Philippine Statistics Authority (PSA) issued Birth Certificate of the Filipino national.
  6. For Divorced, Annulled or Widowed Filipino applicants:
    1. In case of annulment or nullity of marriage or divorce, PSA ISSUED Marriage Contract/Report of Marriage with annotation
    2. In case of death of the previous spouse;
      • If Filipino spouse, PSA issued Death Certificate or Philippine Embassy or Consulate issued Report of Death
      • If foreign spouse, International Death Certificate with Apostille
  7. Affidavit of Delayed Registration of Marriage if marriage was not reported one (1) year after its solemnization.
  8. Confirmed Appointment.

  9. Personal appearance of Filipino spouse.
    Note: The child need not appear during the processing of the Report of Birth unless the parents want to apply for a Philippine passport for him or her, which option must be clearly conveyed through email in the booking of appointment.

  10. Presentation of original documents (submitted by email) on appointment date.
  11. Choose an option on how you want to receive your documents;
    1. By mail: Self-Addressed Envelope with Euro 5.00 stamps
    2. Pick-up (Monday to Friday (except holidays) from 3:00—4:00 pm.
  12. Payment of the required fees in PIN;
    Basic Fees:
    Euro 25.00—(Report of Marriage)
    Euro 25.00—(Affidavit, if Applicable)
  13. Mobile Phone Number.
NOTE:
  1. The applicant may be required to submit additional documents as the Consular Officer may deem appropriate.
  2. The Embassy will forward the Report of Marriage (ROM) to the Philippine Statistics Authority (PSA) in the Philippines. The PSA-certified Marriage Certificate (BC) may be retrieved in the Philippines after 6 months or more from the transmittal filing of the Report of Birth.
