Requirements:
  1. Accomplished Report of Birth application Form.
  2. 2. Uittreksel uit de goboorteakte/International version of the local Birth Certificate from the Local Municipality or “Gemeente”. The Birth Certificate must have an “Apostille” from the appropriate Dutch District Court or “Rechtbank”.
  3. Valid passports of parents and copies of the passports’ data pages
  4. Philippine Statistics Authority (PSA) issued Birth Certificate of the Filipino parents/s.
  5. If parents are married;
    1. PSA issued Marriage Contract or,
    2. PSA issued Report of Marriage issued by the Philippine Embassy or Consulate in the country where the marriage took place or,
    3. Original copy of Report of Marriage (if parents were married or registered in the Philippine Embassy The Hague).
  6. If parents are not married (for child to use the surname of the father);
    1. Affidavit of Admission of Paternity (AAP) to be executed by the father.
    2. Affidavit to Use the Surname of the Father (AUSF) to be executed by the mother or guardian.

      Note: AAP and AUSF should be personally presented and signed by the concerned parent before a Consular Officer at the Embassy.

  7. Certificate of Registration for children born outside of marriage.
  8. Affidavit of Delayed Registration if birth of the child is not registered within one year from the date he/she was born.
  9. Residence Permit(s) of the Filipino parent(s).
  10. If parent(s) is Filipino under RA 9225 (i.e. by the reacquisition or retention of natural born Filipino citizenship), present original Order of Approval of the parent(s).
  11. Confirmed Appointment.

  12. Personal appearance of Filipino parent/s.

    Note: The child need not appear during the processing of the Report of Birth unless the parents want to apply for a Philippine passport for him or her, which option must be clearly conveyed through email in the booking of appointment.

  13. Presentation of original documents (submitted by email) on appointment date.
  14. Choose an option on how you want to receive your documents;
    1. By mail: Self-Addressed Envelope with Euro 5.00 stamps
    2. Pick-up (Monday to Friday (except holidays) from 3:00—4:00 pm.
  15. Payment of the required fees preferably in PIN;
    Basic Fees:
    Euro 25.00—(Report of Birth)
    Euro 25.00—(Affidavit, if Applicable)
    Euro 25.00—(Legal Instrument AAP or AUSF, if applicable)
    Euro 25.00—(Certificate of Registration, if applicable)
    Euro     .10—(photocopy services)

NOTE:
  1. The applicant may be required to submit additional documents as the Consular Officer may deem appropriate.
  2. The Embassy will forward the Report of Birth (ROB) to the Philippine Statistics Authority (PSA) in the Philippines. The PSA-certified Birth Certificate (BC) may be retrieved in the Philippines after 6 months or more from the transmittal filing of the Report of Birth.
