BASIC REQUIREMENTS FOR APPLICATION FOR MINOR CHILD
HOW TO APPLY
- Schedule an Appointment: Log in to https://passport.gov.ph/
- You will receive your Application Form with the confirmed appointment.
- Prepare your documents. Complete your documents before your appointment.
Complete your required documents before your appointment (i.e. Printed copy of application form, et al) Click here for the list of documents required. - Proceed to the Embassy. On the date and time of your appointment, proceed to the embassy’s consular section and bring your documents for submission, further evaluation and payment of consular fees.
- Biometric Data Collection. Your photo and other biometric data (ie. fingerprints) will be taken digitally.
- Passport Release/Collection. Your passport can be collected in person or can be mailed to you for an additional fee.

- Philippine Statistics Authority (PSA)-issued Certificate of Live Birth/ Report of Birth on Security Paper
- Original and one (1) photocopy
- The entries in the passport will follow the Philippine Statistics Authority (PSA) -issued birth certificate/Report of Birth (ie. full name, date of birth, place of birth, sex)
If you do not have a birth certificate issued by the PSA on security paper, please order online through psahelpline.ph or psaserbilis.com.ph or if your prefer an e-apostille copy, please order through www.apostille.gov.ph - If the child is born in the Netherlands, first report the child’s birth to the Embassy. ○ If the child is born outside of the Netherlands, contact the Philippine Embassy/Consulate with jurisdiction on the country of birth of the child for guidance on the requirements and procedure to file the report of birth. (Click here for the directory of Philippine Foreign Service Posts). The Report of Birth will be used for the passport application.
- Proof of Philippine Citizenship
- One (1) original and one (1) photocopy of proof of current Philippine citizenship, such as resident ID card or dual citizenship identification certificate
- Marital Minor Applicants (with married parents as stated in the minor applicant’s PSA-issued Certificate of Live Birth/Report of Birth)
- Valid Philippine passport or other valid identification card of the accompanying parent and the copy of his/her spouse’s passport or other valid identification card;
- If the accompanying parent is non-Filipino, the non-Filipino parent must present his or her foreign passport and a copy of the Filipino parent’s Philippine passport or other valid identification card
- If accompanied by an authorized adult companion, a Special Power of Attorney (SPA) executed by either of the minor’s parents or his/her legal guardian, as the case may be, his/her passport or other valid identification card, and a copy of the passport or other valid identification card of the minor’s parents.
- For Non-Marital Minor Applicants (with unmarried parents as stated in the minor applicant’s PSA-issued Certificate of Live Birth/Report of Birth)
- Only the mother applicant shall accompany the minor applicant in the passport application process;
- A Special Power of Attorney (SPA) executed by the mother will be required if she is not accompanying the minor applicant.
- If the Special Power of Attorney was executed abroad and not before a Philippine Embassy/Consulate, the Special Power of Attorney must be validly authenticated/apostilled;
- Passport or valid identification card of authorized adult companion
- For Foreign Passport Holders
- Original and one (1) photocopy of foreign passport data page (if available)
- Payment
- Euro 60.00 processing fee
