BASIC REQUIREMENTS FOR NEW APPLICATION FOR BY AN ADULT
(also includes those with Brown, Green, Machine Readable (Maroon with no chip) Passports or any older Passports)

How to Apply:

  1. Schedule an Appointment: Log in to https://passport.gov.ph/
    • You will receive your computer generated Application Form with the confirmed appointment.
  2. Prepare your documents. Complete your documents before your appointment. Complete your required documents  before your appointment (i.e. Printed copy of application form, et al).
  3. Proceed to the Embassy. On the date and time of your appointment, proceed to the embassy’s consular section and bring your documents for submission, further evaluation and payment of consular fees.
  4. Biometric Data Collection. Your photo and other biometric data (ie. fingerprints) will be taken digitally.
  5. Passport Release/Collection. Your passport can be collected in person or can be mailed to you for an additional fee.

What to bring:

  1. Passport Application Form
    • Printed Passport Application Form  - You will receive in your email the computer generated Application form with your appointment confirmation
    • Book your appointment here.
  2. Current Passport
    • Original and one (1) photocopy of Philippine passport data page
  3. Philippine Statistics Authority (PSA)-issued Certificate of Live Birth/ Report of Birth on Security Paper
    • Original and one (1) photocopy
    • The entries in the passport will follow the Philippine Statistics Authority (PSA) -issued birth certificate (ie. full name, date of birth, place of birth, sex)

      If you do not have your document issued by the PSA on security paper, please order online through psahelpline.ph or psaserbilis.com.ph or if you prefer an e-apostille copy, please order through www.apostille.gov.ph

      In case you have NO PSA-issued Certificate of Live Birth or Report of Birth

      You must first file for late registration with the Local Civil Registrar (LCR) or Foreign Service Post (FSP) with jurisdiction over the place where the applicant was born.

You need JavaScript enabled to view it. for further information or clarification. The Report of Birth  can be used for the passport application up to one year from the date of registration.

      If you are born outside of the Netherlands, contact the Philippine Embassy/Consulate with jurisdiction on the country of birth of the child. (Click here for the directory of Philippine Foreign Service Posts). The Report of Birth  will be used for the passport application.

  4. Proof of Philippine Citizenship
    • One (1) original and one (1) photocopy of proof of current Philippine citizenship, such as resident ID card or dual citizenship identification certificate.

  5. For Foreign Passport Holders
    • Original and one (1) photocopy of foreign passport data page
  6. Payment
    • Euro 60.00 processing fee
    • Accepted payment methods include cash (exact payment) or  credit/debit card

Supporting Documents for Adult Renewal of ePassport Applications

Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:

Note: Some cases will require the application to be treated as a New Application rather than a renewal.

For a Married Woman Applicant who will Change her Maiden Name to Married Name

  • One (1) original and one (1) photocopy of Philippine Statistics Authority (PSA)-issued Certificate of Marriage/ Report of Marriage

    If you do not have your document issued by the PSA on security paper, please order online through psahelpline.ph or psaserbilis.com.ph or if you prefer an e-apostille copy, please order through www.apostille.gov.ph

  • If married outside the Philippines, first report of marriage to the appropriate Embassy/Consulate. You can use your Report of Marriage for your passport application at the same Embassy/Consulate that issued it, up to one year from the issue date.

For a Married Woman Applicant who  Wishes to Revert to her Maiden Name

  • By virtue of death of the spouse:
    • PSA-issued Certificate of Death or Report of Death (ROD) of spouse or apostillized or authenticated Foreign Death Certificate of foreign spouse with English translation, if applicable;
    • PSA-issued Certificate of Live Birth or Report of Birth;

  • By virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
    • PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
    • PSA-issued Certificate of Live Birth or Report of Birth

  • For other reasons of reversion:
    • PSA-issued Certificate of Live Birth or Report of Birth;
    • PSA-issued Certificate of Live Birth or Report of Birth;
    • Notarized Affidavit of Explanation that includes request for the reversion of maiden name in the Philippine passport or travel document and stating she has not hitherto availed of the reversion;
    • Latest-issued Philippine passport or travel document; and
    • Any existing government-issued valid ID accepted for passport application reflecting the maiden name.

      Note: This mode of reversion can only be done once.

For Dual Citizen Passport Applicants:

  • Natural-born Dual Citizens:
    1. Latest-issued Philippine passport; and
    2. PSA-issued Certificate of Live Birth or PSA-issued Report of Birth.

      Who are natural-born dual citizens?
      Natural-born dual Filipino citizens are individuals who automatically acquire both Philippine citizenship and the citizenship of another country at birth (e.g., by being born to one Filipino parent and one foreign parent).

  • Dual Citizens who availed of the provisions of Republic Act No. 9225:
    1. Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine FSP; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by the Philippine Bureau of Immigration; and
    2. Valid government-issued ID or their secondary citizenship/residence counterparts.
