APPLICATION FOR RETENTION AND RE-ACQUISITION OF PHILIPPINE CITIZENSHIP
UNDER REPUBLIC ACT NO. 9225
REQUIREMENTS
Submit the original and a clear photocopy of the following documents:
- Duly accomplished Petition for Reacquisition/Retention of Philippine Citizenship Form
- Birth Certificate/Report of Birth issued by the Philippine Statistics Authority (PSA)
- Marriage Certificate issued by the PSA — if married.
- Certificate of Naturalization OR Bekendmaking van verlening van het Nederlanderschap
- Official English translation of the Certificate of Naturalization or the Bekendmaking van verlening van het Nederlanderschap
- A sworn translator must do the translation.
- If the applicant has acquired a foreign nationality other than Dutch citizenship, the corresponding Certificate of Naturalization must be apostilled and officially translated into English.
- Dutch or Current foreign passport
- Latest Philippine passport
- Five (5) recent passport-size colored photographs
- White background
- Fee: €50.00 (preferably paid via PIN)
If applicable:
- For widowed applicants:
PSA-authenticated Death Certificate of the deceased spouse or PSA-authenticated Report of Death. - For applicants whose marriage was annulled, declared null, or divorced:
PSA-authenticated Marriage Certificate/Report of Marriage with annotation.
REQUIREMENTS FOR DEPENDENTS 17 YEARS OLD & BELOW
(DERIVATIVE CITIZENSHIP)
Submit the original and a clear photocopy of the following documents:
- Birth Certificate/Report of Birth issued by the PSA
- Apostilled Foreign Birth Certificate — if both parents were no longer Filipino citizens at the time of the child's birth.
- If the Birth Certificate is not in English, submit an official English translation.
- Foreign passport
- Latest Philippine passport
- Three (3) recent passport-size colored photographs for each qualified dependent
- White background
- Fee: €25.00 for each minor child

- Complete and sign the application form.

- Schedule an appointment.
Once the documents have been evaluated, you will be provided with a link to schedule an appointment. - Personal appearance is required.
Pay the applicable fee.- Adult applicant: €50
- Minor child/dependent: €25 per child
- Optional express service: additional €10 for same-day release
- Take the Oath of Allegiance.
If the application is approved, the applicant must take the Oath of Allegiance before a Consular Officer. - Apply for a Philippine passport, if desired.
After taking the Oath of Allegiance, the applicant may apply for a Philippine passport, provided that a separate passport appointment has been booked for the same date. The passport application is subject to the applicable passport requirements and fees.

Important Notes
- Personal appearance is required.
- Bring all the original documents submitted for evaluation.
- Bring and submit two(2) sets of photocopies for all the required documents
- The Embassy may request additional documents if necessary.
- The original PSA Birth Certificate and original translated Bekendmaking will be retained by the Embassy.
- Payment of applicable fees preferably via PIN
